- The purpose of the program is to engage and enhance national leaders and national organizations ability and capacity to expand evidence- and practice-based activities in communities nationwide to address chronic disease risk factors of physical inactivity, poor nutrition, and tobacco use at the policy, systems, and environmental change level in order to reduce the burden of chronic diseases such as, arthritis, obesity, diabetes, cardiovascular disease, and cancer.
