Alzheimer's Disease Programs Initiative: States and Community Program Expansion Grants: Cooperative agreements under this funding opportunity announcement (FOA), Alzheimer's Disease Programs Initiative (ADPI): State and Community Program Expansion Grants, are dedicated to the expansion of promising components within dementia-capable home and community-based service (HCBS) systems projects funded through ACL's Alzheimer's Disease Initiative – Specialized Supportive Services (ADI-SSS) program in 2014, 2015, 2016 and 2017 (HHS-2014-ACL-AOA-AL-0090, HHS-2015-ACL-AOA-AL-0104, HHS-2016-ACL-AOA-ALGG-0152 and HHS-2017-ACL-AOA-ALGG-0227). Promising components would be those components within previously funded programs that would benefit from additional resources to further pilot and demonstrate positive outcomes for persons with dementia and their caregivers. Activities not clearly connected to activities of the originally funded program would not be considered expansions and therefore not eligible for funding under this opportunity.
