Cooperative Agreement for Quality Improvement Development for Part C and Part D Grantees: The purpose of this initiative is to assist Division of Community-Based Programs grantees in developing, implementing, and sustaining a program to track and improve quality of patient care outcomes. This project would provide a defined system for quality improvement, including 1) on-going on-site consultation to at least 175 RWCA Part C and Part D grantees per year, 2) use of specific quality indicators, 3) provision of compatible database software to assist grantees in clinical care data collection and analysis, and 4) collection, aggregation, and analysis of data from grantees to report on clinical quality improvements made on a national level.
