Disparities Elimination through Coordinated Interventions to Prevent and Control Heart and Lung Disease Risk (DECIPHeR) (UG3/UH3 Clinical Trial Optional): This Funding Opportunity Announcement invites applications for projects that propose to test T4 implementation research strategies for delivering proven-effective, evidence-based community-level interventions for reducing or eliminating cardiovascular and/or pulmonary health disparities, and that promote and improve population health in high-burden communities.
