Empowering Older People To Take More Control of their Health Through Evidence-Based Prevention Programs: Summary The Administration on Aging (AoA) will award competitive grants in the form of cooperative agreements to states, as part of a public/private collaboration with the Atlantic Philanthropies, to empower older people to take more control over their own health through lifestyle changes that have proven effective in reducing the risk of disease and disability among the elderly. This collaboration advances President Bush's Healthier US Initiative and Department of Health and Human Service (HHS) Secretary Leavitt's Priority on Personalize Health Care. It also supports and complements the new emphasis on prevention and chronic disease management in Medicare. These grants are designed to mobilize the aging, public health and non-profit networks at the State and local level to accelerate the translation of HHS funded research into practice through the deployment of low-cost evidence-based disease and disability prevention programs at the community level. This initiative holds great potential for improving the quality of life of our seniors and reducing the cost of health care over the long run.

- Eligible Applicants State Aging Agencies and State Health Departments may apply for these grants. Only one application may be submitted by a State. Whichever State agency applies, the proposal must reflect the full commitment and cooperation of the other State agency. This should be reflected in a formal and detailed cooperative agreement or memorandum of understanding attached to the application. The whole proposal, including the work plan, should reflect substantial mutual effort, cooperation, and substantive contributions by each agency throughout the grant period. In addition, a letter of support from the Governor's office is required that indicates support of the project, names the lead agency, and indicates support for a strong partnership effort between the State Health Department and the State Agency on Aging
