Eligible applicants include public and private non-profit entities, including faith-based and community based organizations. All eligible entities applying for funding supported through any of the initiates outlined in the applicant guidance should note that the proposed networks must be comprised of three or more collaborators/members for the duration of the proposed project period. The lead organization/recipient must be a current section 333(e) funded health center.
