High Impact Electronic Health Records Implementation: In support of the Presidents Health Centers Initiative and goal of universal adoption of electronic health records EHR by 2014, this guidance promotes the high impact implementation of an EHR through either a Health Center Controlled Network HCCN or a large individual health center with 30 or more sites; funds must be used for implementation of new EHRs in at least 15 sites. This grant funding opportunity supports the use of EHRs as a tool to improve the safety, quality, efficiency, and effectiveness of health care delivery. The goals of this opportunity include the adoption and effective use of EHRs; the creation of sustainable business models for deploying HIT in HCCNs and large multi-site health centers; enhancing the ability of safety net providers to leverage initiatives and resources as well as improving quality and health outcomes in the Consolidated Health Center Program.

Eligible Applicants Eligible applicants are limited to public and non-profit organizations, including faith based and community based organizations, from one of the following categories: -network controlled by and acting on behalf of the health center(s), as defined and funded under section 330(e)(1)(C) of the PHS Act. At the request of all the member health centers, a network may apply for direct funds if it is at least majority controlled and, as applicable, at least majority owned, by such health centers as defined and funded under section 330(e)(1)(C). For the purposes of this grant opportunity, the term "controlled" means to have the authority collectively to appoint a minimum of 51 percent of the networks board members in the network. If the network is a membership corporation such as a Primary Care Association, then health centers should have a minimum of 51 percent of the membership and if housed within a PCA, provide clear distinct roles of the PCA and Network.; or, -a health center, as defined and funded under section 330 of the Public Health Service (PHS) Act (42 U.S.C. 254b), as amended, applying on behalf of a managed care network or plan, that has received federal grants under subsection 330(e)(1)(A) for at least the two consecutive preceding years; or, -a health center as defined and funded under section 330 of the Public Health Service (PHS) Act (42 U.S.C. 254b), as amended, applying on behalf of a practice management network. Please note that large multi-site health centers must have at least 30 or more sites within their approved scope of project with HRSA.
