Additional Information on Eligibility
Eligible applicants include public and private nonprofit entities. Faith-based and community-based organizations are eligible to apply for these grants. Tribes and tribal organizations are eligible to apply for these grants. Applicants must have significant experience in epilepsy care for children and youth. Applications that fail to show such experience will not be considered. Focus Area 2 Telehealth Disclaimer: Grantees must be a nonprofit or public organizations with a demonstrable successful track record in implementing telemedicine in clinical practices. Applicants are encouraged to have demonstrable telehealth experience and/or documentation that prospective network partners are able to commit to the project. Applicants must submit verifiable information with respect to the commitment of network partners. In addition, an applicant must provide evidence to show that it will be ready to begin to implement the project upon grant award. As cited in 42 CFR Part 51 a.3(a), any public or private entity, including an Indian tribe or tribal organization (as those terms are defined at 25 U.S.C. 450(b), faith-based or community-based organization, that can document previous experience to address and fulfill review criteria in the guidance. Applicants must have experience and be able to document outcomes as a result of their previous activities as they relate to the above "purpose" for these grants.
Grant Description
The purpose of this program is to support innovative strategies in improving access to care for children and youth with epilepsy through one or two of the following approaches: focusing on Mental Health issues for children and youth with epilepsy; implementing telemedicine/telehealth; implementing a training program aimed at educating providers (PCP, Pediatricians) on epilepsy care, particularly in rural and medically underserved areas; partnering with a Community Health Center/ Federally Qualified Health Center or Rural Health Clinic utilizing a chronic health model; and/or through partnering with a disease-specific organization (autism, cerebral palsy, etc) in addressing issues around co-morbidities in an effort to increase awareness about epilepsy.
