Integrated Services for CSHCN: The purpose of this announcement is to fund a National Center on Health Care Transition for Youth with Special Health Care Needs (YSHCN). This National Center will work with the Division of Services for Children and Youth with Special Health Needs (DSCSHN) to develop strategies, policies and programs to assure successful transition for YSHCN from the pediatric to adult system of services. As National Center, this cooperative agreement will: 1) develop a network of experts in transition, including youth leaders, family leaders, State Title V agencies and providers to identify, analyze and expand the current body of data and evidence based practices in the area of health care transition; 2) provide technical assistance to a range of stakeholders, including state Title V agencies, grantees, providers, youth and family organizations to enhance their knowledge and skills on health care transition; 3) utilize quality improvement strategies to bring together stakeholders to work collaboratively on system improvements for YSHCN; 4)work with the pediatric and adult health care providers and professional organizations to support activities that will promote and strengthen practice level changes and enhance their awareness and capacity to serve as a medical home for YSHCN; 5) research, develop and disseminate issues papers, fact sheets, tools and other resource materials and products on health care transition; 6) participate and provide support for Division conferences, grantee meetings, and conference calls and coordinate with other funded National Centers; 7) work with national and state initiatives to promote the importance of health care transition of YSHCN; 8) organize expert meetings and workgroups in the area of transition including convening a forum for timely, interactive communication among key stakeholders; and 9) maintain and update a public website serving as a repository for informational materials and resources.
