Integrating Colorectal Cancer Screening with Chronic Disease: Purpose: The purpose of this one-year funding application is for successful applicants to: a) demonstrate their program’s accomplishment of grantee activities outlined in CDC’s Colorectal Cancer Control Program Logic Model; b) evaluate already identified and already established strategies to achieve the short-term and intermediate outcomes outlined in CDC’s Colorectal Cancer Control Program Logic Model based on continued program activities; and c) complete existing activities that would not otherwise be done because of limited time and funds.
