Only a State Unit on Aging (SUA) may apply for a Nursing Home Diversion Modernization Grant.
Grant Description
Overview: This Program Announcement provides an opportunity for State Units on Aging (SUA), in partnership with Area Agencies on Aging (AAA) and in collaboration with aging service provider organizations and other long-term care stakeholders, to modernize their existing efforts to help individuals who are not eligible for Medicaid to avoid nursing home placement. This opportunity is designed specifically to help the Aging Services Network to transform the way it uses existing funds under the Older Americans Act and other sources to reflect the latest research and best practices in the field and the new long-term care provisions in the Older Americans Act. A State Unit on Aging may propose a project within one or more Planning and Service Areas (PSA). States with a single Planning and Service Area (PSA) may propose a project targeted to a single county or region of the state. By the end of the grant project period (18 months), states must be serving individuals in a manner that is more in line with the standards described in Attachment A of this announcement than they do currently with funds from any one of the following sources: Older Americans Act Titles III-B and/or III-E, Alzheimer's Disease Demonstration Grants to States (ADDGS), or any other program that can be used to serve individuals who are not Medicaid eligible. A state's application will be evaluated in part on the degree of progress it proposes to make in aligning its existing nursing home diversion efforts with the standards in Attachment A.
