Additional Information on Eligibility
This competition is open to Part D grantees with project periods ending July 31, 2011, and new organizations proposing to provide services within the same service areas in place of the current grantee. New organizations must demonstrate that they have the capability to serve the same clients as the existing grantee; that they will serve the same geographic service area; and that they have the capability to serve the target population in a comprehensive, culturally competent, linguistically appropriate, and family-centered manner. In addition, new applicants need to include a transition plan in their application that clearly documents how they will absorb the current grantees' clients, if funding is awarded. A list of current Part D grantees with project periods ending July 31, 2011 and their service areas is included in Appendix B. Eligible organizations are public or private nonprofit entities, including faith-based and community-based organizations, which provide or arrange for primary care services. Applicants are limited to currently funded Part D programs whose project periods expire on July 31, 2011 and new organizations proposing to serve the same populations currently being served by existing projects contained in the application guidance.
Grant Description
The purpose of the Part D funding is to provide family centered care involving outpatient or ambulatory care (directly, through contracts or MOUs) for women, infants, children, and youth with HIV/AIDS. This means all Part D grantees are expected to directly provide or coordinate for the provision of primary medical care to the clients they serve. The projects are expected to conduct case finding in order to bring infected people into care. Once in care, specific efforts must be made to retain individuals in care. Grantees are expected to provide care, treatment, and support services or create a network of medical and social service providers, who collaborate to supply services.
