SPNS Evaluation and Support Center for an Initiative on Enhancing Linkages to HIV Primary Care in Jail Settings: This funding initiative will support one Evaluation and Support Center (Center) to conduct initial information gathering for a potential multi-site evaluation of interventions that target HIV infected individuals transitioning from local jails to the community using innovative models of care to provide HIV primary care and social support services. Specifically, the Center will, in year one, conduct background research on challenges and considerations around the provision of HIV counseling and testing and direct linkages to primary medical care and ancillary services to individuals being held and/or released from local jails. In the following years, the Center will assist demonstration sites in evaluating their innovative methods for providing care and treatment to HIV positive individuals in jail settings who are returning to their communities, locally as well as in a multi-site study.
