Sickle Cell Disease Newborn Screening Follow-Up Program: This announcement solicits applications for the Sickle Cell Disease Newborn Screening Follow-Up Program (SCDNBSFP) to provide support for a Sickle Cell Newborn Screening Technical Assistance Center (TAC) that will improve the care and reduce rates of loss to follow-up of individuals identified with sickle cell disease (SCD) and sickle cell trait through universal newborn screening by supporting efforts of community-based organizations (CBOs) on sickle cell disease education and service coordination. This program will enhance sickle cell CBOs' ability to ensure that individuals diagnosed with sickle cell disease through newborn screening receive appropriate follow-up services including counseling, education, access to a medical home, and other support services. The primary population focus should be families of individuals identified with sickle cell disease and trait through newborn screening.
