Sodium Reduction in Communities: . The purpose of this program is to reduce sodium intake to limits recommended by the Dietary Guidelines for Americans, 2010. Because the majority of sodium that people consume is already present in foods before purchase or preparation, reducing sodium intake may require increased availability and accessibility of lower sodium items. Because community-wide efforts to reduce sodium intake are relatively new, more practice-based evidence would support identification and refinement of effective strategies.

- 1. Eligible Applicants: The following organizations are eligible to apply: a. State Health Departments b. American Indian/Native American Tribal Governments (Federally Recognized) c. County Health Departments (with jurisdictions over 500,000) d. City or Township Health Departments (with jurisdictions over 500,000) 2. Special Eligibility Requirements: State Health Departments: An official state health department (or its bona fide agent), or its equivalent, as designated by the Governor, will serve as the lead/fiduciary agency for Small City/County and Rural Community applications. Each State-Coordinated Application must include at least 2 small city/county or rural community sub-applicants. For this announcement, the term "State" includes the 50 states and the District of Columbia. The term "small city/county" is defined as a local health department that serves a jurisdiction with a population between 50,000 - 500,000 people. The term "rural area" is defined as a local health department that serves a jurisdiction with a population of 50,000 people and below.
