State Implementation Grants for Enhancing the System of Services for Children and Youth with Special Health Care Needs through Systems Integration: This announcement solicits applications for the 2014 State Implementation Grants for Enhancing the System of Services for Children and Youth with Special Health Care Needs through Systems Integration.  The purpose of this initiative is to increase the number of children and youth with special health care needs (CYSHCN) who receive a patient/family-centered medical/health home approach to comprehensive, coordinated services and supports.
