State Implementation Projects for Preventing Secondary Conditions and Promoting the Health of People with Disabilities: CDC?s Procurement and Grants Office has published a funding announcement entitled, ?State Implementation Projects for Preventing Secondary Conditions and Promoting the Health of People with Disabilities.? $2,500,000 [this amount is an estimate, and subject to the availability of funds] will be available in fiscal year 2005 to fund approximately nine to eleven awards. Assistance will be provided to the health departments of States or their bona fide agents or designees, including the District of Columbia, the Commonwealth of Puerto Rico, the Virgin Islands, the Commonwealth of the Northern Mariana Islands, American Samoa, Guam, Federally recognized Indian tribal governments, the Federated States of Micronesia, the Republic of the Marshall Islands, and the Republic of Palau. The purpose of this program is to support States in preventing secondary conditions in persons with disabilities and in implementing effective health promotion and wellness programs for persons with disabilities.
