Technical Assistance to Accelerate the Elimination of Mother to Child Transmission (EMTCT) of HIV and Improve HIV-Free Child Survival in the Republic of Zambia under PEPFAR: The goal and objectives of the program is to solicit comprehensive and cost-effective technical assistance, capacity building and program implementation expertise for PEPFAR and Global Fund-supported global health activities (with emphasis on but not limited to HIV/AIDS) in the five programmatic areas listed under the purpose section of this FOA. This announcement will also support Global Fund implementing partners in assessing HIV service delivery and will develop technical assistance plans and strategies. This FOA will not provide direct program implementation. Rather, it will provide technical assistance support to PEPFAR and Global Fund implementing partners engaged in program implementation and scale-up activities.This FOA will support HHS/CDC’s efforts to provide high-quality, targeted technical assistance to ensure that the PEPFAR and Global Fund-supported countries have the capacity and necessary technical expertise to assume responsibility for service delivery and adequately respond to their HIV epidemics. While this announcement will address several of the overall program goals and objectives, it will primarily focus on five critical PEPFAR-supported technical areas. Applicants are expected to respond to one or more of the following program areas:Prevention of Mother-to-Child Transmission of HIV/AIDS (PMTCT);HIV Care and Treatment Clinical Services for adults and children;HIV Pediatric Care and Treatment (PEDs);Surveillance and Strategic Information (SI); andLaboratoryThis FOA will facilitate the expansion of service delivery support to include coverage of comprehensive Test and Treat implementation in alignment with the latest WHO, PEPFAR and national guidelinestowards the goal of supporting provision of core HIV combination prevention, treatment and laboratory services while assuring quality and sustainability. Activities will be implemented in prioritized geographically locations based on epidemiologic data; in the Southern Province of Zambia.In close partnership with the Government of the Republic of Zambia, the CDC Zambia has made tremendous progress towards achieving an AIDS-free generation. The 2016 Zambia Population Based HIV Impact Assessment (ZAMPHIA) released in December 2016 allows PEPFAR Zambia to better understand Zambia’s epidemic and even better target resources for the greatest impact. This FOA builds on CDC support towards Zambia’s progress towards epidemic control, with a focus on Test and Start, differentiated service delivery models, and addressing programmatic and demographic gaps. Some of the successes of ZAMBIA’s HIV program include: the national launch of Test and Start in late December with firm support from the Ministry of Health; 86% ART coverage according to ZAMPHIA; 12-month retention on treatment of 85%; ZAMPHIA revealed impressive progress towards epidemic control. This FOA will facilitate the expansion of service delivery support to include coverage of comprehensive Test and Treat implementation, as well as expansion of scope of geographic coverage; in alignment with the latest WHO, PEPFAR and national guidelines towards the goal of HIV epidemic control in the Southern Province of Zambia.

Additional Information on Eligibility: CDC Zambia has expanded the scope of the awardee from being a predominantly PMTCT-focused program, to include support for HIV Test and Treat scale up, in accordance with the global and national guidelines for all countries to adopt treatment of all HIV infected persons; these expanded activities will be required at the 120 facilities already allocated to the awardee. This change builds on CDC Zambia’s vision to re-align all site-level support to one partner per site (Re-alignment Phase I), as opposed to multiple partners supporting different aspects of the program along the cascade of prevention, care and treatment, with a goal to attain universal service uptake .Additionally, CDC Zambia has made further significant programmatic shifts to ensure re-alignment of service delivery support to have one CDC partner per district (Re-alignment Phase II) as opposed to multiple partners in a single district); this change will take effect on October 1st 2017 and will involve all facilities/districts being supported by the University of Maryland (UMB), plus an additional 30 facilities ~ totaling 150 facilities within the districts being supported by UMB; as such UMB has been assigned to the majority of thedistricts in Southern Province of Zambia and all targets within the OGAC reporting systems DATIM are assigned to UMB. This expanded scope requires that the awardee deliver on a significant increase intargets across the following program areas, provide more rigorous HIV case finding and an aggressive scale up in placement of individuals living with HIV on treatment. For this reason the awardee, the University of Maryland, Baltimore, has been selected for this sole source award.
