Translating Research Into Action for Diabetes (TRIAD) Legacy Study (U58): The purpose of this research program is to examine health system- and patient-related factors that are associated with long-term processes of care, morbidity, mortality, and quality of life of persons with diabetes; costs; and the reduction of the risks of diabetes, including complications. The focus is on analyzing existing TRIAD study data and examining remaining hypotheses; publishing study findings in peer reviewed journals; and sharing the results broadly through other dissemination mechanisms (web sites; consumer, professional, and business organizations; training programs).

Only applicants who have been funded under CDC Program Announcement No. 04005 (or their successor organization), Translating Research Into Action for Diabetes (TRIAD) are eligible to compete for this funding opportunity. Competition is limited to TRIAD recipients under CDC Program Announcement No. 04005 because they are uniquely positioned to perform, oversee, coordinate, and access the unique existing TRIAD databases. These research institutions include Indiana University Diabetes Research and Training Center; Kaiser Permanente - Oakland, California; Pacific Health Research Institute - Honolulu, Hawaii (or their successor organization); University of California Los Angeles; University of Medicine and Dentistry of New Jersey; and University of Michigan.
