Additional Information on Eligibility
Any urban Indian organization as defined in 25 U.S.C. 1603(h), which has a Board of Directors that is at least 51 percent American Indian or Alaska Native and can demonstrate the Board of Directors is governed primarily by the urban Indian health programs from diverse locations. Only one application per organization is allowed. The following documentation is required: proof of non-profit status and a listing of Board members and their tribal affiliations.
