Guidance for Supplemental CDC/NCCDPHP/BRFSS Funding Opportunity Announcement (FOA) CDC-RFA-DP15-15130201SUPPPPHF16 Behavioral Risk Factor Surveillance System (BRFSS): The Behavioral Risk Factor Surveillance System (BRFSS) is the primary source of state-specific surveillance information about health risk behaviors and health status among the states’ resident population. BRFSS is a state-based system of random digit-dialing telephone and mixed-mode health surveys that target non-institutionalized adults, ages 18 years and older. CDC will work with awardees to develop and implement jurisdiction-specific evaluation plans and ensure accurate evaluation and performance measurement reporting.
