Health Diseaase Collaborative Sustain and Spread: This program will support health centers that are in Phase 2 of a Health Disease Collaborative (HDC) to continue working on sustaining positive changes made durin the first year-long phase of the collaborative and for disseminating the care model throughout the organization, spreading the model to new practice sites and applying it to additional chronic illnesses. Access to HDC quality care management services is critical in ensuringthe overall health adn well-being of the populations served by the Consolidated Health Centers Program. Health Centers that successfully implment the evidence-based model of care through HDC can demonstrate improved primary care practice designs and improved health outcomes for people living with chronic illness.
