Integrated Health and Behavioral Health Care for Children, Adolescents and their Families, Implementation Grants: These three-year implementation grants are designed to operationalize the models of service integration established during a 2-year planning process undertaken by current grantees of this program. The implementation phase will enable the formal working relationships established as part of the planning phase among providers of primary health and medical services comprehensive mental health services and substance abuse prevention and treatment services. The implemented integrated services models are expected to provide for the comprehensive health care needs of a community-based population of children, adolescents and their families and to address the following issues: Organizational structure, governance and leadership; legal and regulatory provisions; functioning in a managed care environment; collaborative working relationships among health professionals from diverse disciplines, including education, training, supervision and work climate; clinica! l practices meeting accepted standards of care and interventions being evidenced based; information systems and protection of patient confidentiality; quality assessment and improvement; relationships with community agencies; and fiscal management and sustainability.
