Opportunity HRSA-11-035: This competing continuation guidance is for "Family To Family Health Care Information and Education Centers for Families of Children with Special Health Care Needs". It is designed to address the President's Health Care Reform efforts to enhance community-based care for Americans with disabilities. This initiative ultimately will assist families so that: "Families of children with special health care needs will partner in decision-making at all levels". Grants will fund state-wide, family-run centers providing information, education, technical assistance and peer support to families of CYSHCN. They will be responsible for developing partnerships with those organizations serving these children and their families and monitor the progress of programs with responsibility for payment and direct services of this population through a statewide data collection system. Specifically, these centers will: 1) Assist families of CSHCN make informed choices about health care in order to promote good treatment decisions, cost effectiveness and improved health outcomes; 2) Provide information regarding the health care needs of and resources available for CSHCN; 3) Identify successful health delivery models; 4) Develop with representatives of health care providers, managed care organization, health care purchasers, and appropriate State agencies, a model for collaboration between families of CSHCN and health professionals; 5) Provide training and guidance regarding the care of CSHCN; 6) Conduct outreach activities to families, health professionals, schools and other appropriate entities; and 7) Be staffed by such families who have expertise in Federal and State public and private health care systems and health providers. Core values across all MCHB programs include family-centered care, family/professional partnerships and cultural and linguistic competence. These MUST be integrated throughout project policies and activities. MCHB will also require additional data and technical assistance. These Centers will report on accomplishments across review criteria and other areas as indicated in this guidance.

- Eligibility for this funding opportunity is limited to organizations within the 41 States that will not have an MCHB-funded Center as of June 1, 2011. Therefore, applicants from the following States only are eligible to apply: AZ, CA, CO, CT, DC, DE, FL, GA, HI, IL, IN, KS, LA, MA, MD, ME, MI, MN, MO, MS, MT, NE, NH, NV, NJ, NM, NY, NC, ND, OK, OR, PA, RI, SD, TN, TX, UT, VA, VT, WA and WI. The remaining ten (10) States (AK, AL, AR, IA, ID, KY, OH, SC, WV, WY), were phased-in during FY 2009, in accordance with the Family Opportunity Act. The F2F HICs in these 10 States are in their second year of funding and will submit a non-competitive progress report to HRSA for FY 2011. These 10 States are not eligible to undergo a new competition until FY 2012, if the F2F HIC Program is continued beyond the ACA extension period.
