Provision of Comprehensive HIV Prevention, Care, Treatment, and Support Programs in the United Republic of Tanzania under the President's Emergency Plan for AIDS Relief (PEPFAR): This NOFO will support the Government of Tanzania’s (GOT) HIV Strategic Plan on comprehensive care and treatment implementation addressing all issues related to the clinical cascade, quality assurance (QA), laboratory, and data management. Recipient(s) will implement innovative, evidence-based, realistic, achievable, measurable, and culturally appropriate comprehensive care and treatment services to achieve the goals of targeted index patient testing to: enhance identification of HIV-positive individuals; enhance linkage to a care and treatment clinic (CTCs) for same day initiation of antiretroviral therapy (ART); retain on treatment and enhance patient adherence to ART while monitoring viral load (VL) to attain viral suppression; and support community-based HIV services to facilitate continuum of care in sustained councils. Recipient(s) will collaborate with regional and district health management teams to implement care and treatment activities at the facility level to achieve the goals of PEPFAR and GOT’s National Multi-Sectoral Strategic Framework IV (NMSF IV). The focus will be on adults, pediatrics, HIV-infected pregnant women and lactating mothers, TB-HIV co-infected patients, key populations (KP), and specific priority populations. Implementation will occur in 16 scale-up and 20 sustained districts within five regions:Kigoma, Pwani, Dar es Salaam, Kagera, and Zanzibar. Districts may be revised over time based on evolving evidence of epidemic burden.
