Well-integrated Screening And Evaluation For Women Across The Nation (wisewoman): The Centers for Disease Control and Prevention, National Center for Chronic Disease Prevention and Health Promotion, Division for Heart Disease and Stroke Prevention, announces the opportunity to apply for funds to implement the Well-Integrated Screening and Evaluation for Women Across the Nation (WISEWOMAN) program. The WISEWOMAN program extends preventive health services to women who are participants of the CDC-funded National Breast and Cervical Cancer Early Detection Program (NBCCEDP). These extended preventive health services include assessment of cardiovascular risk factors and provision of services to reduce those risks through improved diet, physical activity, tobacco cessation, and medication adherence support. A focus on the health systems and community-clinical links that are supportive of these preventive health services is required.

- Eligibility is limited by Congressional statute and applicants must be recipients of National Breast and Cervical Cancer Early Detection Program (NBCCEDP) funding. WISEWOMAN is authorized under NBCCEDP to expand screening services inclusive of cardiovascular risk. WISEWOMAN program participants must be eligible for NBCCEDP to receive WISEWOMAN services; therefore, only organizations who are currently receiving NBCCEDP awards may apply. These include: State governments or their Bona Fide Agents (this includes the District of Columbia) Territorial governments or their Bona Fide Agents (this includes Commonwealth of Puerto Rico, the Virgin Islands, the Commonwealth of the Northern Marianna Islands, American Samoa, Guam, the Federated States of Micronesia, the Republic of the Marshall Islands, and the Republic of Palau) American Indian/Alaska Native tribal governments (federally recognized or state-recognized) American Indian/Alaska Native tribally designated organizations
