Fiscal Year 2005 Competitive Application Cycle for Service Area: Competition for Consolidated Health Center Program (CHCP): PURPOSE The Health Resources and Services Administration?s (HRSA) Bureau of Primary Health Care (BPHC) administers the Consolidated Health Center Program, as authorized by section 330 of the Public Health Service (PHS) Act, 42 U.S.C. 254b, as amended. The Consolidated Health Center Program promotes the development and operation of community-based primary health care service systems in medically underserved areas and improves the health status of medically underserved populations. This application guidance details the Service Area Competition (SAC) eligibility requirements, review criteria and awarding factors for organizations seeking a grant for operational support under the Consolidated Health Center Program including: Community Health Centers (CHC), Migrant Health Centers (MHC), Health Care for the Homeless (HCH), Public Housing Primary Care (PHPC) and School Based Health Centers (SBHC) authorized under section 330 of the PHS Act as amended.

- ELIGIBLE APPLICANTS An application will be considered eligible if it meets all of the specific eligibility requirements. Applications that do not meet the eligibility requirements will not be accepted for processing and will be returned. Organizations eligible to compete include public or nonprofit private entities, including tribal, faith-based and community-based organizations. All applicants are expected to demonstrate compliance with the requirements of section 330 of the PHS Act as amended. Interested organizations should refer to the HRSA Preview and/or Federal Register for further information regarding specific areas and deadlines. Existing grantees whose project periods expire on or after October 31, 2004 should apply for grant funding under this guidance. New organizations proposing to serve the same area(s) and/or population(s) currently being served by these existing centers may also apply for a grant for operational support under section 330 of the PHS Act as amended under this guidance. Organizations are also eligible to apply for areas designated in the HRSA Preview and/or Federal Register as currently being served by interim grantees under this guidance.
