Alzheimer's Disease Programs Initiative - Grants to States and Communities (Round Two): Alzheimer's Disease Program Initiative (ADPI) cooperative agreements are dedicated to the development and expansion of dementia-capable home and community-based service (HCBS) systems in States and Communities. There will be two application options through this opportunity, one for States (Option A) and the other for Communities (Option B). No entity would be eligible to apply for both State and Community options. The systems resulting from program activities under both program options will provide quality, person-centered services that help individuals remain independent and safe in their communities.
