As cited in 42 CFR Part 51 a.3(a), any public or private entity, including an Indian tribe or tribal organization (as those terms are defined at 25 U.S.C. 450(b), faith-based or community-based organization, that can document previous experience to address and fulfill review criteria in the guidance. Applicants must have national experience and be able to document outcomes as a result of their previous activities as they relate to the above "purpose" for this cooperative agreement.
Grant Description
The purpose of this announcement is to fund a National Center for Community Based Services for Children and Youth with Special Health Care Needs. This National Center will work with the Division of Services for Children and Youth with Special Health Needs (DSCSHN) to assure that services provided at the community level are integrated and easy to use for families of children and youth with special healthcare needs. As a National Center, this cooperative agreement will: 1) collect, analyze, and expand the current body of data and evidence-based practices and models of community based services and disseminate those findings; 2) provide technical assistance and support to a range of stakeholders working to develop, improve and spread successful models of community based services, including state Title V agencies, grantees, providers, youth and family members and organizations; communities and other stakeholder groups; 3) use continuous quality improvement strategies to promote and spread system change at the community level; 4) develop and implement a community recognition program that focuses on successful community integration models; 5) identify successful models of community integration in the areas of information technology; partnership development and coalition building; innovative financing; local service provision; state and community capacity and resource development; 6) research, develop and disseminate issues papers, fact sheets, tools and other resource materials and products; 7)participate and provide support for Division conferences, grantee meetings, and conference calls and organize expert meetings and workgroups to support the core outcome that community-based services for CYSHCN are organized so families can use them easily; 8) maintain and update a public website serving as a repository for informational materials and resources; and 9) coordinate with other funded National Centers and grant programs.
