Additional Information on Eligibility
The applicant must be a public or non-profit entity that represents a network that includes at least three or more health care providers. In addition, the grantee must meet the following requirements: The applicant organization must be located in a eligible rural area or in a rural census tract of a metropolitan county (list included in application materials and on program Web site www.ruralhealth.hrsa.gov/funding/eligibility.htm. All grant funded activities must support rural areas. Existing networks that seek to expand services or expand their service area are not eligible to apply for this program. Public and non-profit private entities, including faith-based and community organizations. Applicants in rural counties or designated eligible census tracts within an urban county. Applicant providing services exclusively to migrant and seasonal farm workers in rural areas and is supported under Section 330G of the Public Health Service Act. Federally recognized Native American Tribe or Tribal Organization that will deliver services on a Reservation or federally recognized Tribal lands.
