Consortium of National Networks to Impact Tobacco-Related and Cancer Health Disparities: Purpose: Through a collaborative FOA, CDC seeks to fund a consortium of public health oriented national networks to impact tobacco-related and cancer health disparities within specific target populations. The target populations are: 1). African Americans; 2). American Indians/Alaskan Natives; 3). Asian Americans/Pacific Islanders/Hawaiian Natives; 4). Latinos/Hispanics; 5). Lesbian, Gay, Bisexual and Transgender persons; 6). Persons with Low Socioeconomic Status; 7). Persons with Mental Health and Substance Abuse Disorders; and 8). Geographically Defined Populations with High Commercial Tobacco Use and Related Health Disparities. This tailored strategy is needed to achieve the benefits of an overall population-based approach to commercial tobacco use prevention and cancer prevention and control and the achievement of health equity for all populations.

- Eligibility Information 1. Eligible Applicants: - American Indian/Alaska Native tribal governments (federally recognized or state-recognized) - American Indian/Alaska native tribally designated organizations - Alaska Native health corporations - Colleges - Community-based organizations - Faith-based organizations - For-profit organizations (other than small business) - Hospitals - Nonprofit with 501C3 IRS status (other than institution of higher education) - Nonprofit without 501C3 IRS status (other than institution of higher education) - Political subdivisions of States (in consultation with States) - Research institutions (that will perform activities deemed as non-research) - Small, minority, and women-owned businesses - State and local governments or their Bona Fide Agents (this includes the District of Columbia, the Commonwealth of Puerto Rico, the Virgin Islands, the Commonwealth of the Northern Marianna Islands, American Samoa, Guam, the Federated States of Micronesia, the Republic of the Marshall Islands, and the Republic of Palau) . - Tribal epidemiology centers - Universities - Urban Indian health organizations 2. Special Eligibility Requirements: Applicants must demonstrate the ability to meet ALL of the following special eligibility requirements: - In order to effectively support, disseminate, and amplify public health efforts to address tobacco-related and cancer health disparities, the applicant organization must provide evidence of national reach as evidenced by a. An organizational chart that includes member affiliates and locations to reflect established organizational reach in at least five (5) NCCDPHP regions. See http://www.cdc.gov/coordinatedchronic/docs/NCCDPHP-Regions-Map.pdf , as well as: b. Letters of support from at least two (2) affiliate members from each NCCDPHP regions identified in the applicant's organizational chart. This evidence must be uploaded in Grants.gov under "Other Attachment Forms", as a separate attachment and must be labeled "Evidence of National Reach". Failure to submit evidence in the manner described will result in the application being considered non-responsive and will not be entered into the review process.
