Core II: The purpose of the program is to strengthen the ability of state health department injury prevention programs to address: A. older adult fall prevention, B. child injury prevention and C. teen dating violence prevention. To this end, this FOA is divided into three distinct parts: Part A, Part B and Part C. Applicants may apply for one, two or all parts and may be funded for one or multiple parts. This program addresses the "Healthy People 2010" focus area(s) of Injury and Violence Prevention.

Eligible applicants that can apply for this funding opportunity are listed below: Current state department of health recipients of the Public Health Injury Surveillance and Prevention Program Funding (05027) or their bona fide agent (See Attachment 1). A Bona Fide Agent is an agency/organization identified by the state as eligible to submit an application under the state eligibility in lieu of a state application. If applying as a bona fide agent of a state or local government, a letter from the state or local government as documentation of the status is required. Attach with "Other Attachment Forms" when submitting via www.grants.gov.
