NWD/ADRC Opportunity - Sustainability Competive Continuation: The U.S. Administration for Community Living (ACL), the Centers for Medicare & Medicaid Services (CMS) and the Veterans Health Administration (VHA) have created this funding opportunity for ADRC grantees to continue their ADRC/NWD system efforts to help people learn about and access the long term services and supports (LTSS). This funding opportunity would provide additional funding and an additional 12 months for ADRC grantees to carry out their current activities under their Part B - ADRC Sustainability Program Expansion Supplemental awarded in FY 2012. This funding opportunity is specifically designed to help support states efforts to continue their sustainability strategies for ADRC/NWD System in conjunction with their health systems transformation and funding from the Center for Medicare and Medicaid (CMS) and Veteran Health Administration (VHA). Under this opportunity, States will continue to work toward developing a high performing statewide ADRC Options Counseling Program as part of their ADRC/NWD system as outlined in their own statewide development/expansion plans and as described in the 2012 ACL, CMS, VHA Funding Opportunity “The Enhanced Aging and Disability Resource Center Options Counseling Program.” Successful applicants will be provided an additional 12 months to continue to seek the following two key sustainability goals as part of this funding opportunity: Strengthening the capacity of their ADRC/NWD system to serve people of all ages, income levels and disabilities by adopting a “No Wrong Door” approach that operationally involves a wide array of community agencies and organizations so it can effectively serve all populations; and Developing financially sustainable ADRC/NWD models that includes revenue from multiple public programs (including Medicaid, Medicare, the Older Americans Act, the Veterans Health Administration and other programs).
