Service Coordinators in Multifamily Housing (SCMF) Discretionary: The Service Coordinators in Multifamily Housing (SCMF) program supports service coordinator positions for elderly individuals and non-elderly persons with disabilities living in HUD assisted housing. Service coordinators play a critical role in connecting older adults and persons with disabilities with community-based supportive services for independent living and reducing premature and unnecessary transitions to higher levels of care. Service Coordinators work to promote access to resources, financial security, social connections, health and well-being for residents in assisted housing. Service coordinators help residents identify and access supportive services that will enable them to continue living independently in the community and age in place. Participation in the service coordinator program is voluntary, and residents choose which services they accept.
