Sickle Cell Disease Program: The purpose of this grant activity is to fund demonstration projects to address issues regarding newborn screening and follow-up of infants with Sickle Cell disease and trait babies. These projects will provide models, best practices, and dissemination strategies for ensuring optimal follow-up and management of babies identified with Sickle Cell disease and/or traits. This grant activity is to support the comprehensive care for newborns diagnosed with Sickle Cell disease or trait and their families, relying on partnerships among the State Title V and newborn screening programs, community-based Sickle Cell disease organizations, comprehensive Sickle Cell disease treatment centers, and community-based primary care professionals.
