Special Diabetes Program for Indians Community-Directed Program: The purpose of this Indian Health Service cooperative agreement is to provide diabetes treatment and/or prevention activities and/or services (also referred to as "activities/services") for AI/AN communities. Grantees will implement one Special Diabetes Program for Indians Diabetes Best Practice (also referred to as "Best Practice") and report data on its Required Key Measure. Grantees may also implement other activities/services based on diabetes-related community needs and develop an evaluation plan. Activities/services will be aimed at reducing the risk of diabetes in at-risk individuals, providing high quality care to those with diagnosed diabetes, and/or reducing the complications of diabetes.

- To be eligible for this "New/Competing Continuation Announcement" under this cooperative agreement announcement, applicants must be one of the following:
i. A Federally-recognized Indian Tribe as defined by 25 U.S.C. 1603(14), operating an Indian health program operated pursuant to a contract, grant, cooperative agreement, or compact with Indian Health Service (IHS) pursuant to the Indian Self-Determination and Education Assistance Act (ISDEAA), (Pub. L. 93-638).
ii. A Tribal organization as defined by 25 U.S.C. 1603(26), operating an Indian health program operated pursuant to a contract, grant, cooperative agreement, or compact with the IHS pursuant to the ISDEAA, (Pub. L. 93-638).
iii. An urban Indian organization, as defined by 25 U.S.C. 1603(29), operating a Title V urban Indian health program that currently has a grant or contract with the IHS under Title V of the Indian Health Care Improvement Act, (Pub. L. 93-437). Applicants must provide proof of non-profit status with the application, e.g. 501(c)(3).
iv. Indian Health Service facilities: Under this announcement, only one Special Diabetes Program for Indians (SDPI) Community-Directed diabetes grant will be awarded per entity. If a Tribe submits an application, their local IHS facility cannot apply; if the Tribe does not submit an application, the IHS facility can apply. Tribes that are awarded grant funds may sub-contract with local IHS facilities to provide specific clinical services. In this case, the Tribe would be the primary SDPI grantee and the Federal entity would have a sub-contract within the Tribe's SDPI grant. - Link to Full Grant Announcement
