State Implementation Grants for Integrated Community Systems for CSHCN: The purpose of this initiative is to support statewide implementation of the HRSA component of the Presidents New Freedom Initiative to create inclusive community based systems of services for children and youth with special health care needs. Under this initiative HRSAs Maternal and Child Health Bureau has the lead in developing and implementing a plan to achieve appropriate community based service systems, as defined by the following components family professional partnership comprehensive health care through a medical home access to adequate health insurance early and continuous screening organization of community services for easy use by families and transition to adult health care work and independence. Applicants must be or work closely with the State Title V Program for Children with Special Health Care Needs have a complete and current statewide needs assessment that addresses the States status in each of the 6 systems components, work with partners to develop and implement a statewide plan to achieve a community based system of services for CYSHCN using evidenced based and best practices models recommended by the Maternal and Child Health Bureau, integrate the plan into the Title V Block Grant and other public private programs serving CYSHCN and their families, define a comprehensive evaluation plan using national state and community data and demonstrate the capacity to gather and report on systems outcomes related to system outcomes for CYSHCN, identify, recruit and lead teams which will participate in learning collaboratives designed to improve access to a medical home and demonstrate the capability to sustain the project beyond Federal funding.
