Telehealth Network Grant Program: Telehealth Networks TNGP-TH Grants made under this provision will support telehealth networks that provide services in different settings e.g., long-term care facilities, community health centers or clinics, physician offices, hospitals, schools, assisted living facilities, homes. To demonstrate how telehealth networks can be used to: a. expand access to, coordinate, and improve the quality of healthcare services; b. improve and expland the training of healthcare providers; and/or c. expand and improve the quealtiy of health information available to healthcare providers, patients, and their families. Telehomecare Networks TNGP-THC Grants made under this provision are focused on demonstrating how telehealth networks can improve healthcare through provision of clinical care and remote monitoring of patients in thier place of residence using telehealth technologies. TNGP-THC projects provide a mechanism to evaluate cost-effectiveness of telehomecare services and may include, but are not limited to, case management by physicians, hospitals, medical clinics, home health agencies, or health care providers who supervise the care of patients in their homes.

Additional Information on Eligibility
See eligibility section in Guidance. The applicant can be either rural or urban nonprofit entity that will provide services through a telehealth network TNGP-TH or a telehomecare network TNGP-THC. Proof of non-profit status will be included with the application. Each entity participating in the networks may be a nonprofit or a for-profit entity. Faith-based and community based organizations are eligible to apply. Services must be provided to rural communties, although the applicant can be located in an urban area. Composition of the Network The TNGP-TH shall include at least two (2) of the following entities (at least one of which shall be a community-based health care provider. -Community or migrant health centers or other federally qualified health centers. -Health care providers, including pharmacists, in private practive. -Entities operating clinics, including rural health clinics. -Local health departments. -Nonprofit hospitals, including community (critical) access hospitals. -Other publicly funded health or social service agencies. -Long-term care providers. -Providers of health care services in the home. -Providers of outpatient mental health services and entities operating outpatient mental health facilities. -Local or regional emergency health care providers. -Institutions of higher education. -Entities operating dental clinics. -Other entities may be included in the network (e.g., state or local correctional facilities); however, they shall be counted as one of the two requires sites listed above.
