Health Center Consortia Opportunity: The purpose of this program is to expand access to primary health care for the medically underserved by creating new primary care delivery sites and expanding existing health services. Health center grantees will collaborate with other health providers (such as hospitals, public clinics, free clinics or other health care service organizations) in their communities to increase access, improve referral systems, enhance provider communication and coordination, and reduce the inappropriate use of hospital emergency rooms. The primary purpose of this funding opportunity is to expand access to primary health care. As part of the proposed budget for that primary purpose, funds awarded under this opportunity may be used to enhance or develop information technology systems of collaborating 330-funded health centers in order to improve service delivery.
