Real Choice Systems Change Grants for Community Living: The Centers for Medicare & Medicaid Services (CMS) is inviting proposals for grants totaling up to $35 million of these funds. States and others, in partnership with their disability and aging communities, may submit proposals aimed at building infrastructure that will result in effective and enduring improvements in community long-term support systems. These systemic changes are designed to enable children and adults of any age who have a disability or long-term illness to: a) Live in the most integrated community setting appropriate to their individual support requirements and preferences; b) Exercise meaningful choices about their living environment, the providers of services they receive, the types of supports they use and the manner by which services are provided; and c) Obtain quality services in a manner as consistent as possible with their community living preferences and priorities. People of any age who have a disability or long-term illness, as do most other Americans, generally express the same desire to live in the community. They express a desire to live in their own homes, make their own decisions about daily activities, work, learn, and maintain important social relationships. They express a desire to contribute and participate in their communities and family life. Several grant opportunities comprise this round of "Real Choice Systems Change Grants for Community Living" and are the subject of this invitation to apply; they are titled: Feasibility Studies and Development Grants 1. Respite for Adults 2. Respite for Children 3. Community-Based Treatment Alternatives for Children Research and Demonstration Grants 4. Quality Assurance and Quality Improvement in Home and Community-Based Services 5. Independence Plus Initiative 6. Money Follows the Person Rebalancing Initiative 7. Community-Integrated Personal Assistance Services and Supports Technical Assistance to States, State Advisory Committees and Families Grants 8. National State-to-State Technical Assistance Program for Community Living 9. Technical Assistance for Consumer Task Forces 10. Family-to-Family Health Care Information and Education Centers Grant applications are due on July 29, 2003. All grant awards will be made prior to October 1, 2003. Grantees will have up to 36 months to expend these funds. Grantees are required to make a non-financial recipient contribution of five percent (5%) of the total grant award (including all direct and indirect costs). Non-financial recipient contributions may include the value of goods and/or services contributed by the grantee (i.e., salary and fringe benefits of staff devoting a percentage of their time to the grant not otherwise included in the budget or derived from Federal funds).
